INSTRUCTIONS FOR GOLDFARB PATIENT SAFETY AND QUALITY
FELLOWSHIP APPLICATION
( for admission in 2009)

We accepted applications for our first class of fellows from October, 2007 through January 15, 2008 with a start date of July, 2008.  Our second cohort of fellows will start July, 2009. 

The fellowship program cycles with the academic year (July to July).  Applications are reviewed by the patient safety fellowship steering committee.  Please check your eligiblity to be sure you are able to apply.  (At this time, this fellowship is not available for physicians who work at St. Louis Children's Hospital.)

CHECKLIST
-Completed application form (2 pages)
-Applicant Career Development Plan (2-5 pages)
-Applicant Research Plan (2-5 pages)
-Current NIH Biosketch, including "other support" and bibliography
-Mentors (existing/proposed/need help)
-Letter from Division Chief and/or Program Director, attached to this application
-Two letters of reference sent directly to the program office (letters may also be emailed to patientsafety@im.wustl.edu, provided they are on official letterhead, signed, and come directly from the reference:

Please be sure you have provided all necessary information for the application. Font must be 11 points or larger (Arial or Helvetica 11 point is preferred). Type must not exceed 6 lines per inch. Please do not staple the pages. Minimum half inch margins.
